AVIATION TRAINING & CONSULTING SERVICES
Studies show that more than 80% of aviation accidents involve human error rather than pure mechanical or technical causes (human error, active or latent, underlies most aviation accidents & incidents). From a flight operation perspective, it has been found that crew errors include complacency, mode confusion, cognitive overload, complex decision making under high stress, interpersonal communication issues etc. (Flight crew error can be operationally defined as “crew action or inaction that leads to deviation from crew or organizational intentions or expectations” Helmreich, Klinect &Wilhelm, 1999).
Multiple positive approaches have evolved over time to establish countermeasures that focus on human performance issues and error reduction strategies. Awareness programs such as Human Factors (HF), Crew Resource Management (CRM) and Threat and Error Management (TEM) were subsequently introduced and are now well established programs within the aviation industry. In most countries such programs are a mandatory regulatory requirement. Additionally, the introduction of Safety Management Systems ensures high-level management accountability for flight safety while aiming for a realistic and efficient balance between safety and profitability.

”What CRM means to us... the application of human factors knowledge and skills to accomplish the objectives of safety, efficiency and management of risk. In a nutshell, Katie and her team delivered an effective, informative and tangible training course that engaged, provoked and incited a clear awareness and understanding of this complex and intriguing subject.” ~ Barry Collier, Managing Director / Director Operations / Line Captain, Jet Aviation Business Jets, Hong Kong
"Katie is a great trainer. She always works with the operator to customize the training, tailor it to address the operator’s specific needs so participants receive a high quality and valuable training session, not just another ‘cookie cutter’ training in the industry. Katie is extremely knowledgeable in Aviation Safety and Human Factors. I know I can always count on her for advice on Human Factors training strategies and resources and she’s always open and willing to help. I’m so happy and honored to have had the opportunity to work with Katie and that our relationship has grown from customer/service provider to good friends and remains close professionally and personally even after I relocated. I strongly recommend Katie and her exceptional team to anyone who’s looking aviation safety/human factor training for corporate/commercial aviation." ~ Joanne Wong, Director of Quality, Security, Environment, Health and Safety, Jet Aviation Flight Services East, USA.
I have been working with OneCrew for 8 consecutive years now and the CRM courses that Katharine developed have been the best I have undertaken for decades.
As business jet operator, most of the CRM courses that have been proposed to us were mainly directed to and for airlines, OneCrew has been the only provider in Asia that has managed to adapt the CRM to business jet operators specific and provide very valuable and interactive courses that have been innovative and lateral thinking in her approach to various subjects. I cannot thank Katharine and her OneCrew team enough for their help in keeping our operation and Crew more professional, and our situation awareness regarding the safety of the Operation wide open. ~ Patrick Tardieu, Chief Pilot, Vision Values Aviation

“Mission accomplished! OneCrew’s Train-the-Trainer Workshop has consistently provided our trainers and evaluators with the tools, knowledge and skills necessary to perform our jobs effectively and confidently. This has had a tangible effect on the credibility of our Training Department in the eyes of trainees, trainers and evaluators as well as the regulatory agency. Did I mention also that Katie and her team managed to make the week of training absolutely fun?” ~ Ivan Luciani, Chief Training Captain, Metrojet, Hong Kong
I am extremely grateful to Katie for bringing out the best in me during her 2 year tenure as Cebu Pacific’s Human Factors Advisor/Consultant. She was able to mold me, with much precision, to become a good Human Factors facilitator and later, was able to urge me, guide me, support me in becoming a Crew Resource Management program developer (the new 'best practice' program is now being delivered to the entire cabin crew, flight deck crew and flight dispatch community). Katie’s unending support and guidance has given me more strengths in being a facilitator and her magic made me discover my own magic. All of these are doors that she opened for me, and it paved way to me becoming Cabin Crew of the Year for 2018. And I thank Katie for being a huge part of that! ~ M.J. Aquino, Human Factors Facilitator & Cabin Crew Supervisor, Cebu Pacific Air, Philippines
Katie is, without doubt, very experienced and knowledgable Human Factors/Crew Resource Management trainer, team coach and consultant in the aviation industry. Her technical and non-technical understanding with her years of broad expertise makes Katie undoubtedly the best trainer we ever had at Cebu Pacific. We are truly grateful to be trained by the best! ~ Gladys Guevarra, Human Factors Officer, Cebu Pacific Air, Philippines
