AVIATION TRAINING & CONSULTING SERVICES
Studies show that more than 80% of aviation accidents involve human factors considerations such as human error, rather than pure mechanical or technical causes. From a flight operation perspective, it has been found that crew errors include complacency, mode confusion, cognitive overload, complex decision making under high stress, interpersonal communication issues etc. (Flight crew error can be operationally defined as “crew action or inaction that leads to deviation from crew or organizational intentions or expectations” Helmreich, Klinect &Wilhelm, 1999).
Multiple positive approaches have evolved over time to establish countermeasures that focus on human performance issues and error reduction strategies. Awareness programs such as Human Factors (HF), Crew Resource Management (CRM) and Threat and Error Management (TEM) were subsequently introduced and are now well established programs within the aviation industry. In most countries such programs are a mandatory regulatory requirement. Additionally, the introduction of Safety Management Systems ensures high-level management accountability for flight safety while aiming for a realistic and efficient balance between safety and profitability.

Katharine Ng
Founder & Managing Director, OneCrew Ltd.
Katharine Ng, a leading specialist in Aviation Human Factors, empowers business, commercial and general aviation to prioritize safety. She heads OneCrew Aviation Training Services in training leadership, pilots, cabin crew, dispatchers and engineers on essential Crew Resource Management skills and Aviation Security awareness, fostering strong safety and security culture.
Previously, Katharine shaped Cebu Pacific Air's human factors safety programs through initiatives aimed at transforming safety culture. She also co-founded Viva Macau Airlines, where she led the Human Resources and Inflight Services Departments. Katharine’s aviation journey started as a flight attendant for Singapore Airlines and Cathay Pacific Airways, to 8 years with Cathay Pacific’s Human Factors Department as project leader.
A passionate advocate for aircrew mental health and psychological safety in the workplace, Katie continues to raise awareness and empower the aviation community—driving meaningful change, and inspiring broader conversations across industries. She is the author of The Ordinary Entrepreneur: From Flight Attendant to Business Owner, available on Amazon.
Brian Legge
Senior Consultant & Human Factors Specialist, OneCrew Ltd.
Brian Legge is a Training Captain at Cathay Pacific Airways flying the Boeing 777. He is also one of Cathay’s most experienced CRM facilitators, a Flight Data Analyst on their FDAP team, the Regional Vice President of the International Federation of Air Line Pilots’ Associations (IFALPA), and an ICAO AVSEC Professional Management course instructor.
In 2014, Brian joined OneCrew- a specialist corporate & aviation training consulting company, as Senior Consultant, where he develops and delivers management skills and CRM training for general aviation and business aviation clients.
Brian was born and raised in Vancouver, Canada. His aviation career spanned across Canada; including postings in British Columbia, Saskatchewan, Ontario, and Nova Scotia before moving to Hong Kong, where he has resided for the last decade. Brian has accumulated over 15,000 hours in fixed wing aircraft over 20 years; he holds type ratings on the Boeing 777, 747, and several turboprop aircraft.
Brian has a long history of training. As a Class 1 Flight Instructor, he was the Chief Flight Instructor of a successful Canadian Aviation College. Over the last 15 years, Brian has designed and presented Human Factors and Crew Resource Management (CRM) courses for a variety of audiences including: airline, business jet, and regional carrier crews.
Brian holds a Bachelor of Arts degree in Economics and Psychology from the University of British Columbia and an Executive MBA with a specialization in Aviation Management from Concordia University in Montreal. He is also a member of the Beta Gamma Sigma Honours Society
Julian Hipwell
Managing Director, OneCrew Australia Pty. Ltd.
Julian Hipwell is a pioneer in the development of Human Factors and Crew Resource Management (CRM) and helped introduce CRM practices to the aviation community in Asia.
In 2004, Julian founded OneCrew Australia and joined OneCrew Hong Kong as Director of Program Development, where he leads CRM training and Human Factors consulting projects for commercial, business and general aviation. Julian is a qualified safety and lead auditor. An active contributor in the arena of aviation and medical human factors, Julian has presented at numerous international safety conferences.
Julian's background includes 8 years as Crew Resource Management Program Manager for Cathay Pacific Airways. Julian trained and maintained a facilitation staff of 50 to conduct all facets of CRM and Human Factors training. The team of pilots and cabin crew facilitators provided CRM training to thousands of pilots and cabin crew of Cathay Pacific Airways. Other initiatives include the production and development of a three-day CRM introductory course for pilots and flight attendants. In addition, development initiatives such as LOFT (Line Orientated Flight Training), a simulator-based non-jeopardy training and the LOSA (Line Operational Safety Audit) program, a production of a series of training videos based on airline-specific events and the introduction of the ‘Take Care’ program for cabin crew to reduce in-flight incidents.
An aviation and safety enthusiast, Julian has 34 years of experience as a pilot in both private and commercial airplanes.
In loving memory of OneCrew co-founder, a world-class management skills trainer and coach, a generous and loving man who lived life to the fullest, and on his own terms. His happiest day was in a training room connecting with participants and making a difference.

As part of that commitment AAPA plans to hold the next Asia Pacific Aviation Safety Seminar (APASS) on 10-11 September 2025 in Manila, Philippines. The host Airline for APASS 2025 is Philippine Airlines.
APASS is the premier regional aviation safety event and is designed to create a common meeting place for the region's airlines, airports, aircraft manufacturers, insurers, ground handlers, MROs and suppliers to discuss and to be updated on the latest developments and priorities in aviation safety.
